This form is needed for all individuals under the age of 18 who would like to be pierced. This form is to be filled out by parent/guardian and notarized by a public notary. TIP: Parent/guardian will need a state issued photo ID to have form notarized and to be pierced. Parent/guardian who is on notary form MUST be present at time of piercing with ID, clients who are 17, may have parent/guardian leave once piercing paperwork is completed and piercing is approved.
